Audit Forms


General Information

The audit forms described below should be used to audit contractor compliance with, and supervisor awareness of, the requirements of the Contractor Safety Manual. Separate forms are also included to document and report audit results.

The forms are Microsoft Word documents that can be printed and filled out by hand, or completed electronically.

Form Description and Use

Safety Compliance Audit (GSK-SA-07)

  1. Use this form to document whether or not the work being performed by the contractor complies with the requirements of the Contractor Safety Manual.
  2. The form provides a large selection of administrative (A) and Technical (T) questions. An audit is performed by choosing ten questions, five administrative and five technical.
  3. Each contract should be audited at least once every month.
  4. Auditors are required to follow up on all violations observed and record the date when the corrective action occurred.

Safety Compliance Audit Summary (GSK-SA-08)

  1. Use this form to record the calculated results of twelve audits.
  2. Blank spaces are provided to indicate the type of question asked (administrative or technical) and the number of the question selected from the list.
  3. Auditors should not ask the same question repeatedly without justification.

Supervisor's Safety Awareness and Compliance Audit (GSK-SA-09)

  1. Use this form to document whether or not the contractor's supervisor is aware of the requirements of the Contractors Safety Manual and if the supervisor is complying with those requirements.
  2. The form provides a large selection of administrative (A) and Technical (T) questions. An audit is performed by choosing ten questions, five administrative and five technical.
  3. Each contract should be audited at least once every month.
  4. Auditors are required to follow up on all violations observed and record the date when the corrective action occurred.
