The following forms should be used to document safety related information. They are Microsoft Word documents that can be printed and filled out by hand, or completed electronically.
Alternate forms, such as forms developed by contractors, may be used if approved by GlaxoSmithKline (GSK).
EHS-0022 - Mobile Crane Evaluation
GSK-SA-01 - Report of Accident, Environmental Release, Property Damage, or Near Miss
GSK-SA-05 - Safety Procedure Waiver
